CCME is committed to maintaining the trust and confidence of our participating physician practices and visitors
to our DOQ-IT website. We have created this web policy to demonstrate our firm commitment to your privacy and the
protection of your information.
Personal Information
You do not have to give personal information to visit the public portion of the DOQ-IT website. This public
section gives visitors the chance to read and obtain more information on the DOQ-IT project and CCME. Additional
activities on our site require you to be a registered participant of the DOQ-IT project in order to access the
Members Only section.
In order to access the Members Only DOQ-IT website, you will be required to apply to participate in the DOQ-IT
program. Once your physician practice has been accepted into the DOQ-IT project, a unique username and password
will be assigned to your practice. This unique username and password will be used to log in to the Members Only
site and will track your movement through our site.
As part of the registration process, you will be asked to provide information about the physician practice, the
physicians in this practice, and the DOQ-IT project contact leader.
Our primary purpose in collecting personal and practice information is to provide you with a safe, smooth,
efficient, and customized experience. This allows us to provide services and features that will best meet your
needs, and to customize our service to make your experience safer and easier. We only collect personal information
about you that we consider necessary for achieving this purpose.
Our Use of Your Information
We use your personal information to facilitate the services you request. We use your personal information in
the file we maintain about your practice, and other information we obtain from your current and past activities on
the site to troubleshoot problems; measure consumer interest in our services; inform you about online and offline
services and updates; customize your experience; detect and protect us against error, fraud and other criminal
activity; and as otherwise described to you at the time of collection. We may compare and review your personal
information for errors, and omissions and for accuracy.
When you apply, you agree that we may use personal information about your practice to analyze site usage,
improve our content, and customize the site's content, layout, and services, and to improve our marketing efforts.
These uses improve the DOQ-IT site/project and tailor it to better meet your needs, so as to provide you with a
smooth, efficient, safe and customized experience while using the DOQ-IT website.
Collection of Information
Personally identifiable information or personal information is defined as information that identifies your
physician practice, physicians in your practice and the project contact leader. It includes the name of your
physician practice; names of the physicians in your practice and their UPIN numbers; the project contact
leader’s name; and the physician practice’s and project contact leader’s addresses, telephone numbers, fax numbers
and e-mail addresses.
We only collect personally identifiable information about your practice after you have specifically and
knowingly provided such information to us. By voluntarily providing this information to us during the application
process, you will be consenting to the collection and use of your personal information by CCME.
Disclosure of Your Personal Information
If you provide personal information, such as your mailing or e-mail addresses, telephone or fax numbers, or
other demographic information, CCME will not share (disclose or sell) it to third parties. This information is
strictly for use for the DOQ-IT project and will be used only by CCME staff.
Non-Personal Information
We analyze traffic to this website in order to understand our members’ and visitors' needs and to continually
improve our site for them. The DOQ-IT website automatically collects and records non-personally identifiable
information, which is information that does not identify you personally, but becomes available to us as a result
of your visit to our site. This information includes: date and time of visit, pages visited on our site, type of
computer and browser the visitor is using, and the location of visitor’s machine (by country, not street address).
Many websites gather this information, though most do not notify visitors. We use this information to improve our
services to you and do not share it with third parties.
DOQ-IT Cookies
A cookie is a small piece of data that is sent to your Internet browser from a web server and stored on your
computer’s hard drive. A cookie cannot read data off of your computer hard disk or read cookie files created by
other websites. Cookies do not damage your system. You can choose whether to accept cookies by changing the settings
of your Internet browser. You can reset your browser to refuse all cookies, or allow your browser to show you when
a cookie is being sent. CCME will not be able to assist you with changing the setting on your computer, so that it
may accept our cookie.
Certain applications on www.thecarolinascenter.org/doqit require session cookies to function correctly. If you have session
cookies disabled, you may not be able to use these applications or features of our site.
Where they are used, DOQ-IT's session cookies and/or session variables remember your selection criteria. You do
not need to have session cookies enabled to view static web content on www.thecarolinascenter.org/doqit.
We have set our software so that your browser will only return cookie information to the domain where the cookie
originated (in this case, www.thecarolinascenter.org/doqit). No other site can request it.
Note: Regardless of the particular uses for cookies on the DOQ-IT website, we will not share any cookie
information with any third parties.

Electronic mail is not secure against interception. If your communication to us is very sensitive, we recommend
sending it by postal mail instead of e-mail. Our postal address is 100 Regency Forest Drive, Suite 200, Cary, NC

Why did you receive an e-mail from us?
By accepting the Web Policies, you expressly agree that we may use your personal information to contact you and
deliver information to you that, in some cases, is targeted to your interests.
If you received a mailing from us, (a) your e-mail address is either listed with us as someone who has expressly
shared this address for the purpose of receiving information in the future (“opt-in”), or (b) you have registered or
otherwise have an existing relationship with us. We respect your time and attention by controlling the frequency of
our mailings.

Bulletin Board
If you wish to participate in the DOQ-IT Bulletin Board, please remember that any information disclosed will be
available for viewing by the other members/users of this community. Users should exercise caution when deciding to
disclose personal or personally identifiable information.
